Clinical Pharmacist responsibilities include:
1. Review and verify medication orders for appropriateness, dosage, and potential interactions
2. Conduct comprehensive medication reconciliation during care transitions
3. Provide drug information and evidence-based recommendations to physicians and nurses
4. Monitor patient response to therapy and adjust regimens as needed
5. Participate in multidisciplinary rounds and contribute to treatment planning
6. Manage anticoagulation, antimicrobial stewardship, and other clinical pharmacy services
7. Document clinical interventions and maintain accurate patient medication records
8. Educate patients on proper medication use, side effects, and adherence strategies
9. Oversee sterile and non-sterile compounding operations ensuring USP compliance
10. Implement medication safety initiatives and quality improvement projects
1. Doctor of Pharmacy (Pharm.D.) degree from accredited institution
2. Current state pharmacist license in good standing
3. Completion of ASHP-accredited PGY1 pharmacy residency
4. Board Certification in Pharmacotherapy (BCPS) or eligibility
5. 2+ years of clinical pharmacy experience in hospital setting
6. Proficiency with EHR systems (Epic, Cerner) and clinical decision support tools
7. Advanced knowledge of pharmacokinetics, pharmacodynamics, and pharmacogenomics
8. Demonstrated ability to manage complex medication regimens for critically ill patients
9. Strong interdisciplinary collaboration and communication skills
10. ACLS certification and immunization certification
Preferred Qualifications
1. PGY2 residency specialization in critical care, oncology, or infectious diseases
2. Experience with antimicrobial stewardship program implementation
