Must-Have Requirements

1. High school diploma or GED equivalent 2. Completion of state-approved CHW training program (minimum 100 hours) 3. Valid driver's license and reliable transportation 4. Basic knowledge of local community resources and health services 5. Bilingual proficiency in English and at least one other language commonly spoken in the service area 6. Minimum 1 year of experience working in community-based settings 7. Understanding of cultural norms and health beliefs within diverse communities 8. Ability to maintain confidentiality and professional boundaries 9. Strong interpersonal and communication skills 10. Basic computer literacy for documentation and data entry

Preferred Qualifications

1. Associate's or Bachelor's degree in Public Health, Social Work, or related field 2. Certification as a Community Health Worker (if required by state) 3. 2+ years of experience in community health outreach 4. Experience with electronic health records (EHR) systems 5.
