Community Health Worker responsibilities include:
1. Conduct outreach to community members to connect them with appropriate health and social services
2. Provide culturally appropriate health education and information to individuals and groups
3. Assist community members in accessing healthcare services, including scheduling appointments and arranging transportation
4. Advocate for individual and community health needs within healthcare and social service systems
5. Collect data and maintain records of services provided and community health outcomes
6. Facilitate communication and understanding between community members and healthcare providers
7. Provide basic health screening services and referrals under supervision
8. Organize and participate in community health fairs and wellness events
9. Conduct home visits to assess living conditions and provide follow-up care

1. High school diploma or GED equivalent
2. Completion of state-approved CHW training program (minimum 100 hours)
3. Valid driver's license and reliable transportation
4. Basic knowledge of local community resources and health services
5. Bilingual proficiency in English and at least one other language commonly spoken in the service area
6. Minimum 1 year of experience working in community-based settings
7. Understanding of cultural norms and health beliefs within diverse communities
8. Ability to maintain confidentiality and professional boundaries
9. Strong interpersonal and communication skills
10. Basic computer literacy for documentation and data entry
Preferred Qualifications
1. Associate's or Bachelor's degree in Public Health, Social Work, or related field

3. 2+ years of experience in community health outreach
4. Experience with electronic health records (EHR) systems
5. Knowledge of Medicaid and Medicare programs
